- Do NOT patch: Severe hydrogen blistering, stress corrosion cracking (SCC), or general wall thinning over >30% of the component. For general thinning, the vessel must be de-rated or replaced.
- Do NOT patch: Vessels in Lethal Service (per UG-2 of ASME VIII) without special variance from the National Board.

2. Flush Insert Patch (The "Button" Patch)
- Description: The damaged section is cut out completely. A new, perfectly fitted piece of steel is inserted flush with the existing shell and welded with a single-V or double-V groove.
- Advantage: Smooth internal surface (no protrusion).
- ASME Requirement: Requires 100% radiography of the butt weld. This is the gold standard.
2. Addenda (The "Service Pack")
